This page has various documents needed by those going on the Italy trip.
Important Forms
The following documents are waiver and release forms. They are all MS Word documents you can fill out, print, sign, and return. Send an email if you would like these documents in a different form.
- District 509J Waiver - needs to be signed by student and parent.
- Statement of Informed Consent, Authorization, and Release - A form for passport information and medical details and release. Needs to be signed by student and parent with copies of passport by Jan. 16th.
- Camerata Italy Code of Conduct - needs to be signed by student and parent.

Various Informational Doucments
They following dociments have more information. Each document is a zip archive containing an MS Word document:
- Roster - A list of students on the traveling ensemble and accompanying adults.
- Payment Information - A letter with information about payment and dates for each installment payment. The payment dates are also listed on the calendar.
- Fund Raising - A list of fund raising plans. Interested donors can also be directed to the Camerata PayPal donation site (link the "Support Our Trip" section on the right), where they can donate on-line using a credit card or a their own PayPal account.
